Palliative Care Program
Individuals eligible for Bethany’s Palliative Care Program include those living with a life-limiting illness who feel they may benefit from additional assistance navigating their healthcare options and who are not yet ready for hospice.
As a consultative program, Bethany works with the patient’s established medical team to improve quality of life. Patients can continue to receive curative treatment and skilled benefits throughout the consultation process.
Our palliative consults focus on:
goals of care discussions
advance care directive & POLST completion
community resource planning & referrals
insurance benefit review & enrollment in complimentary palliative care programs
caregiver support
hospice education
coordination of care between treating physicians & service providers
Palliative consults are provided at no cost to patients.
We can help establish treatment goals and preferences, determine advance care plans, support family caregivers, and facilitate needed community- or insurance-based resources.
Conditions such as cancer, COPD, heart disease, kidney disease, liver disease, Alzheimer’s dementia, Parkinson’s, and ALS may benefit from a palliative care consult.
