PRN position: Hours are flexible as is convenient to the employee. All visits must be completed in a timely fashion.


Provide skilled professional nursing care to patient/family/caregiver as prescribed by the physician and in compliance with all applicable laws, regulations, and company policies and procedures.  Provide  face to face visits to patients and assess for hospice recertification in accordance with medical guidelines.  Implement the plan of care to meet patient/family/caregiver needs, initiating appropriate interventions upon admission.  Re-evaluate the patient’s nursing needs to provide appropriate, comprehensive and responsive care plan interventions.  Provide timely documentation of patient services based upon the plan of care.  Work under the direction of the Director of Clinical Services and with the interdisciplinary team in the planning, implementation and evaluation of medical and nursing care.  Report off to the RN Case Manager for each patient seen.  Manage related expenditures in a fiscally responsible manner in accordance with the company budget, travel as necessary.





1.      Seek and obtain sufficient information necessary to function effectively in the setting where the patient/family/caregiver requires care.

2.      Conduct nursing assessment, including physical and psychosocial aspects of patient/family/caregiver in a thorough, accurate, compassionate and timely fashion.

3.      Provide report to the RN Case Manager and to the care team to assist in developing an individualized plan of care to include problem identification, interventions and goals for palliative comfort.

4.      Contribute to identifying both active and potential needs of the patient/family/caregiver which may affect the plan of care.

5.      Provide face to face patient visits in a timely fashion in accordance with all pertinent regulations.

6.      Communicate in writing in the chart and verbally with the team to coordinate interventions.

7.      Facilitate and participate in the care conferences and contribute to the patient care planning as assigned by the Director of Clinical Services.

8.      Facilitate implementation of the care plan interventions and provide direct patient care as necessary.

9.      Perform within the limits of preparation, training and experience.

10.  Work in cooperation with the patient/family/caregiver and the interdisciplinary team.

11.  Complete and maintain all patient records at patient’s residence, maintain chart and submit accurate and relevant notes regarding patient’s condition.

12.  Assist with field training as needed.

13.  Function with little supervision, utilizing time constructively and organizing daily assignments for maximum productivity.

14.  Communicate clearly and appropriately to patient/family/caregiver, physicians, nursing facility staff and other referral sources, relating all pertinent information in written and verbal forms.  Maintain open communication with supervisory personnel.

15.  Maintain clinical records, statistics, reports and records for reporting and evaluating purposes in compliance with local, state and federal laws.

16.  Participate in staff meetings, department meetings, team meetings, briefings, in-services, committees and other related activities as needed.

17.  Assure compliance with local, state and federal laws, Medicare regulations and established company policies and procedures.

18.  Meet or exceed delivery of company service standards in a consistent manner.

19.  Conduct all business activities in a professional and ethical manner.

20.  Maintain all appropriate state licensing requirements.

  1. CRNP licensed in the state.
  2. Minimum of one year of nursing experience.  Hospice, home health, skilled nursing facility or medical, surgical, or critical care experience and experience with pain and symptom management interventions preferred.
  3. Understanding of hospice philosophy and needs of the terminally ill.
  4. Compliance with accepted professional standards and practices.
  5. Physical exam in accordance with state law requirements.
  6. Ability to work in an interdisciplinary setting.
  7. Satisfactory references from employers and/or professional peers.
  8. Satisfactory criminal background check.
  9. Presentation of negative TB skin test and/or CXR and other tests as required by hospice and/or state law.
  10. Valid driver’s license and an automobile that is insured is accordance with state law and is in good working order.
  11. Demonstrate excellent observation, good nursing judgment and communication skills.  Ability to provide good written documentation in a timely manner.
  12. Self-directed with the ability to work with little supervision.
  13. Flexible and cooperative in fulfilling all obligations.
